Tenders are invited for the supply of services in respect of a Hospital Discharge Rapid Response Service.

Hospital Discharge Rapid Response Services ensure that patients are able to return to their own homes safely when they no longer require treatment and care in hospital. These services tackle ‘delayed discharge’ and ensure that customers who have been in hospital are able to return home quickly with the range of social support that is required to help them to regain independence, health and well being.

The service will address the following Priority Outcomes for Northamptonshire County Council:

• We are here to help you when you can’t help yourself
• We want you to see us as a trusted advocate

The service will support the delivery of the Northamptonshire Prevention Strategy, ‘Supporting Independence; A Strategy for the Development of Prevention and Early Intervention Services 2009-2011’ and the revised Prevention Strategy to be published during 2011. It will also support the delivery of priority outcomes for NHS partners.
